All appointment scheduling services are facilitated through Acuity Scheduling, an independent third-party booking platform. All clinical services are provided solely by independent ADHS-registered Arizona providers (MD, DO, NP, or PA) who are not employees, agents, or representatives of this Site.
When you book an appointment through this Site, you are scheduling with an independent, third-party Arizona-licensed provider registered with ADHS under Proposition 203. These providers are solely responsible for:
We do not supervise, direct, credential, control, or employ any provider. No provider–patient relationship exists between you and SurpriseMarijuana.Clinic.
We do not guarantee that you will receive a written certification, that ADHS will approve your patient card application, or that an appointment will be available at a particular time or price. Certification decisions are made solely by the independent provider. Patient card approval decisions are made solely by ADHS.
Use of this Site is intended for adults who are 18 years of age or older and who are Arizona residents with a qualifying condition recognized by ADHS under Proposition 203. Consult the ADHS Medical Marijuana Registry for current eligibility rules.
Appointment booking is facilitated through Acuity Scheduling. Payment may be made by major credit card at the time of booking. Refund eligibility is governed by the policy disclosed at booking and/or the independent provider's posted policy.
The ADHS state fee ($150 per year, or $75 for financial hardship) is paid directly to ADHS through the ADHS Medical Marijuana Registry — not to this Site. That fee and its refund eligibility are governed entirely by ADHS policy.

You may not use this Site for any unlawful purpose, to interfere with Site security or functionality, to submit false or misleading information, or to harm others.
